Underwriting Guidelines
Underwriting decisions are determined based on the applicant’s responses to health questions and their build:
- Level Plan (Preferred): If the applicant answers “No” to all questions in Parts A, B, and C, they may be eligible for the Level Death Benefit Plan as a Preferred health risk.
- Level Plan (Standard): If the applicant answers “No” to all questions in Parts A and B but “Yes” to one or more in Part C, they may qualify as a Standard health risk under the Level Death Benefit Plan.
- Modified Plan: If the applicant answers “No” to all questions in Part A but “Yes” to one or more in Part B, they may qualify for the Modified Death Benefit Plan.
- Automatic Decline: Any “Yes” answer to a question in Part A or a build that falls outside the acceptable chart will result in a decline.
Health Questions for SBLI Final Expense
The following health questions are part of the underwriting process for SBLI’s Living Legacy® Final Expense product. These questions help determine eligibility for the Level or Modified Death Benefit plans and whether the applicant qualifies for Standard, Preferred, or Modified underwriting classes.
Part A – Health Questions
- Are you currently, or in the last 6 months, undergoing treatment for any medical condition?
- Do you require a wheelchair due to a chronic illness?
- In the last 5 years, have you received home care, or been treated in a hospital or psychiatric facility?
- Have you ever been diagnosed or treated by a medical professional for cancer, heart disease, stroke, or diabetes?
- In the past 2 years, has a licensed medical professional advised you to take medication or undergo tests?
- Have you been hospitalized by a licensed medical facility in the last 90 days?
Part B – Medical History
- Have you ever tested positive for HIV, hepatitis, or another infectious disease?
- Are you currently receiving Kidney Dialysis?
- Have you ever been diagnosed with mental health conditions, including depression, bipolar disorder, or schizophrenia?
- Except for traffic violations, in the last 5 years, have you been convicted of a felony or misdemeanor?
Part C – Medical Treatment
- Have you had surgery or other major treatments within the past 5 years?
- Do you take any prescribed medications regularly?
- Have you had an amputation, including finger, leg, or limb replacement due to injury or illness?
Underwriting Decision Guidelines
- Level Plan (Preferred): If the applicant answers “No” to all questions in Parts A, B, and C, they may be eligible for the Level Death Benefit Plan as a Preferred health risk.
- Level Plan (Standard): If the applicant answers “No” to all questions in Parts A and B, but answers “Yes” to one or more questions in Part C, they may qualify as a Standard health risk under the Level Death Benefit Plan.
- Modified Plan: If the applicant answers “No” to all questions in Part A, but answers “Yes” to one or more questions in Part B, they may qualify for the Modified Death Benefit Plan.

Applicants who answer "yes" to disqualifying questions or fall outside build/rx tolerances will be declined or shifted to a lower benefit tier (e.g., from Level to Modified).

Payment Details

This is a permanent whole life product. There are no conversion options available. Once issued, the plan remains in force for life as long as premiums are paid.
