Product Overview and Available Plan Types
CICA Life offers two permanent whole life insurance plans under the product line names Choice Standard and Choice Guaranteed Issue (GI). These plans are designed to provide simplified, guaranteed coverage to individuals between the ages of 0 and 85, with varying face amount limits depending on age and plan type. Both plans provide permanent protection with level premiums and nonforfeiture options.
Issue Ages and Maximum Face Amounts
- Ages 0–50: $1,000 to $30,000 (both plans)
- Ages 51–70: Standard: $1,000 to $20,000 | GI: $1,000 to $30,000
- Ages 71–85: $1,000 to $10,000 (both plans)
The Choice Standard plan offers an immediate death benefit, while the Choice GI plan pays out 110% of premiums in the first two years, then transitions to full coverage in year three.
Riders Included and Optional
Both plans include the following riders:
- Terminal Illness Accelerated Death Benefit
- Accidental Dismemberment Benefit
Optional for $5/month flat fee:
- Accidental Death Benefit: Doubles face amount (max $30,000); available ages 0–84; ends at 85
Underwriting Guidelines and Health Conditions
Choice Standard uses simplified underwriting with:
- No height/weight requirements
- No tobacco questions
- No MIB or Rx history checks
Physician information is encouraged but not required.
Applicants answering "yes" to any of the following are offered GI instead:
- Hospitalization, hospice, or assistance with ADLs
- HIV/AIDS, recurrent/metastatic cancer
- Stroke, CHF, uncontrolled diabetes, heart disease
- Neurological diseases: Alzheimer’s, ALS, dementia, etc.
- Substance abuse, felony, suicide attempt (last 2 years)
- Organ transplant within 5 years
Medication Restrictions
No formal prescription check. Eligibility based on answers to health questions and whether applicant is under physician care with stable medication use.
Common Health Conditions and Eligibility
- AIDS, dementia, cystic fibrosis: GI only
- CHF, COPD, pacemaker, obesity: Standard if stable
- Cancer: GI if current or recurring; Standard if past & resolved
- Insulin-dependent diabetes: Standard if controlled
- Organ transplant (over 5 years): Standard
- Oxygen use, mental health (under care): Standard
Conversion Options
No conversion features available. These are permanent plans.
Premium Payments and Billing Guidelines
- Payment: EFT, credit/debit, Direct Express, or direct bill
- Modes: Monthly, Quarterly, Semi-Annual, Annual
- Initial premium must be paid within 2 days
- Premiums lock based on upload date, not first draft
Nonforfeiture Options
- Reduced Paid-Up: Must request within 60 days
- Extended Term: Default option if no request made

Accidental Dismemberment Benefit Details
Pays 33% of face amount for qualifying injuries. Must occur within 90 days of the accident. Ends at age 85. Covered events include:
- Loss of limbs, sight, thumb/index finger, or combinations
Exclusions: Self-harm, DUI, criminal acts, military service, aviation
Commissions and Advance Rules
50% advanced for EFT; remainder paid months 7–12. Other modes are paid as-earned. Weekly payout on Fridays for settled business by Tuesday 5 p.m.
Chargebacks apply if lapse occurs within 6 months.
Override Commission Restrictions
Not allowed for unlicensed individuals in certain states including: FL, GA, KY, LA, MA, MT, NM, NY, NC, PA, SC, SD, TX, VA, WV, WI
