Riders – Ensured Legacy Final Expense

Royal Neighbors of America offers several valuable riders to enhance the core life insurance coverage. Some riders are included automatically, while others must be elected at the time of application. All riders are subject to availability by product type and state. Full details are outlined in the Rider Brochure.

Included Rider

  • Accelerated Death Benefit – Terminal Condition and Permanent Confinement (Form Series 221203):
    • Provides a one-time lump sum payment of a portion of the certificate's death benefit if the insured is diagnosed with a qualifying terminal illness or is permanently confined to a nursing home.
    • Qualifying events:
      • Terminal condition with a life expectancy of 12 months or less as certified by a physician.
      • Continuous confinement to a qualified nursing home for 90 days or more, expected to continue until death.
    • Max acceleration: 75% of face amount or $100,000 (whichever is less); minimum benefit is $5,000. Admin fee: up to $100.
    • Available on SIWL and GDB only. Not available on GI.
    • Only one acceleration per insured; rider available only at issue; rider eligible for reinstatement.
    • Benefit may be taxable and affect eligibility for public assistance. Not assignable. No additional premium required, but lien and interest will apply.

Optional Riders

  • Accidental Death Benefit Rider (Form Series 222140):
    • Pays full rider face amount if death occurs within 180 days of an accidental bodily injury.

Availability: e-App can be completed in person or remotely. The Insured, Owner, and Payor can be different individuals and do not need to be in the same location.

  • Height/Weight is only required for Preferred rating. Face amount must be $25,000 or higher for Preferred.
  • Trusted Contact Person and Second Addressee options are available.
  • Separate Owner allowed with insurable interest. See acceptable ownership rules (pages 15–17 of agent guide).
  • Separate Payor allowed; Social Security benefit-tied draft date supported.
  • Same Day draft available; Save Age options supported by adjusting draft date prior to birthday.
  • e-Signature methods: Email, SMS text, or Voice. Electronic or voice signature accepted for disclosures.

e-App Interview Steps

  1. Pre-qualify applicant and Proposed Insured.
  2. Access and complete the e-App.

Pre-Qualify the Applicant

All applicants must be pre-qualified before submitting an application for SIWL, GDB, or GI coverage with Royal Neighbors of America. Agents are expected to follow these steps:

  1. Verify Identity: View a valid government-issued photo ID such as a current driver’s license, state ID, or passport. If unable to verify identity, this must be noted in the Agent’s Report section during the application interview.
  2. Confirm Eligibility: Individuals who are not eligible for coverage include:
    • Anyone without a Social Security number
    • Anyone who is not a U.S. citizen or permanent resident (Green Card holder)
    • Anyone mentally incompetent or otherwise unable to legally contract
    • Any applicant or owner with a felony conviction (regardless of face amount)
    • Anyone who already owns $40,000 in SIWL, $20,000 in GDB, or $10,000 in GI with Royal Neighbors
    • Anyone with three or more lapsed certificates with Royal Neighbors
  3. Use the POS Agent Worksheet: Complete Form 2416-A to prepare for the phone interview and reduce delays.
  4. Review Medical Questions: Use Section 6 of the application (Form 231115 or state-specific versions) along with the Risk Assessment Guide.

Certificate Delivery

All insurance certificates will be mailed directly to the certificate Owner.

Reinstatement

To reinstate a lapsed certificate, the owner must pay all past-due premiums necessary to bring the policy current. Requirements and procedures for reinstatement are defined in the Insurance Certificate.

Re-date Policy

A re-date updates the certificate’s effective date to a new current date, with premium payments beginning as of the new date. Royal Neighbors allows one re-date per certificate, and only within the first 60 days after issue. Submit re-date requests to the Underwriting Department at (309) 788-4561, Option 1, Option 1, Option 1.

All Whole Life products from United Home Life / United Farm Family Life use simplified issue underwriting. There are no medical exams or fluids. Underwriting is based on yes/no health questions, MIB, prescription database, and in some cases, a Personal History Interview (PHI).

  • Must have valid driver’s license. No more than two moving violations in past three years. No DUI in past five years.
  • No excessive use of alcohol, history of alcohol abuse or treatment.
  • Not an active member of the military.
  • Must have legally resided in US for the past three years.
  • Must be employable on a full-time basis; cannot be receiving any form of disability compensation.
  • Aviation: No participation as a pilot or crew member in the past two years.
  • No ratable avocation (i.e., racing, scuba diving, mountain climbing, rodeo, sky-diving).

Preferred Criteria Ages 66–80 (Only for Provider Whole Life)

Previously listed criteria plus all of the following:

  • No history of fractures or falls in past 10 years.
  • Must be able to perform Activities of Daily Living (ADLs) without assistance:
    • Feeding
    • Bathing
    • Dressing
    • Taking own medications
    • Toileting
    • Transferring (mobility)
    • Telephone use, shopping, housekeeping, yard work
  • Must answer an additional set of supplemental health questions during the PHI (Personal History Interview).

Supplemental Health Questions for Ages 66–80 (Only for Provider Whole Life)

All preferred underwriting criteria must be met along with a total of five (5) points from answers to the following questions. Each “yes” answer is 1 point. Questions are asked during the PHI interview:

  • Does the Proposed Insured exercise a minimum of two days per week, for a total of one hour per week?
  • Does Proposed Insured have a valid driver’s license, and still drive?
  • Is the Proposed Insured currently employed for a minimum of 10 hours per week?
  • Does the Proposed Insured currently volunteer for a minimum of two hours per week?
  • Does the Proposed Insured live with a spouse or roommate, or have pet ownership?
  • Does the Proposed Insured participate in recreational travel, hobbies, or sports?
  • Does the Proposed Insured handle all financial transactions independently without requiring outside assistance to pay bills and perform normal banking transactions?

Preferred is available only for Provider Whole Life applicants who meet strict standards. These include:

  • No ratable medical conditions
  • Must have a current MD and active health management
  • No treatment for high cholesterol or high blood pressure
  • No mental illness or depression in the last 10 years
  • No bankruptcy or criminal conviction in past 10 years
  • No family history of cardiovascular disease or cancer death in parents/siblings before age 60
  • Valid U.S. driver’s license, no more than 2 moving violations in 3 years, and no DUI in 5 years
  • No alcohol abuse or treatment history
  • Not active-duty military
  • U.S. residency for 3+ years
  • Must be fully employable; not receiving disability income
  • No aviation or hazardous hobbies (racing, diving, climbing, rodeo, etc.)

Preferred Criteria for Ages 66–80

  • All previous Preferred criteria must be met
  • No history of falls or fractures in past 10 years
  • Must perform all Activities of Daily Living (ADLs) unassisted

Activities of Daily Living (ADL) Required:

  • Feeding

Attending Physician Statements (APS)

An APS may be required based on the Proposed Insured’s:

  • Medical history
  • MIB (Medical Information Bureau) findings
  • Pharmaceutical (Rx) history

APS Guidelines:

  • Agents will be notified by the underwriter if an APS is needed
  • The underwriter will specify the physician and time frame for the records
  • Clients are responsible for requesting the APS from their provider
  • The APS cost is paid by the client

WAIVER OF PREMIUM (TOTAL AND PERMANENT
DISABILITY: Premiums waived during total and permanent disability up to the policy anniversary nearest the Insured’s 60th birthday. See Page 18 for rates per thousand. Rider terminates if requested in writing, on the date the policy is continued under a non-forfeiture option, if any, or on the policy anniversary nearest the Insured’s attained age 60.

CHILD RIDER: Provides coverage for children issue ages 30 days – 18 years (age last). Sold in units of $5,000 (max 4 units); premium is $25 per unit. Child must be insurable. Separate application required. Fully convertible prior to child’s attained age 21 (or date rider terminates, if earlier)
without evidence of insurability. Child Rider can be added to a grandparent’s base policy only if the child has been legally adopted by the grandparent. Rider terminates if requested in writing, if the Insured commits suicide within two years of the rider effective date, the date the policy is continued under a non-forfeiture option, if the policy is at the end of its premium paying period, or on the policy anniversary at the Insured’s attained age 65. While the rider is in force, insurance on any child terminates on the child’s 21st birthday. First $5,000 of  Child Rider coverage is at no charge for base policy face  amounts of $100,000 or more.

ACCIDENTAL DEATH BENEFIT RIDER: Provides a benefit in addition to the base policy benefit should the Insured die by accidental bodily injury independent of all other causes,
provided death occurs within 90 days of such injury. See Page 18 for rates per thousand. Rider terminates at the earliest of the policy anniversary nearest the Insured’s 70th birthday, the date the policy is continued under a non-forfeiture option, or if requested in writing. Minimum face amount for base policy and ADB rider is $25,000 each. Max ADB rider not to exceed lesser of base policy face amount or $200,000.

TERMINAL ILLNESS ACCELERATED BENEFIT: Provides
the Policyowner with the right to access the policy’s death benefit (discounted at interest for one year) on the life of the Insured if the Insured is diagnosed with a life expectancy of 12 months or less.

This product is ideal for clients who want their premiums returned at the end of a 20-year level term period if they are still living. Return of Premium Benefit

If the insured survives the 20-year level premium period, and no death benefit has been paid, the policy will return 100% of base premium paid to the policyowner. This is a tax-free return of premium benefit under current tax laws.

Global Underwriting Rules – All Term Products

Non-Tobacco Definition

No nicotine replacement, smoking or tobacco products including, but not limited to: nicotine gum, patch or pills, cigarettes, cigars, chew, pipe, e-cigarettes or vape within the past 12 months.

Medical Examinations / Bodily Fluids Testing

No routine exams or lab testing required. United Home Life / United Farm Family Life reserves the right, however, to order such requirements at the underwriter’s discretion.

Attending Physician Statements (APS)

Based on the Proposed Insured’s medical history, MIB information, or pharmacy report, an APS may be necessary. Agents will be asked to provide the APS at the client’s expense. APSs are accepted only if provided by the physician/medical provider in a sealed envelope or faxed directly from the provider’s office.

Foreign Nationals

Applications are accepted on individuals who are not naturalized U.S. citizens provided they are in the U.S. legally and on a permanent basis. The applicant must possess a Social Security number, a valid Visa or Green Card, and have resided in the U.S. for the past two years.

Military Risks

Applications are accepted on individuals currently serving in the military in a non-combat unit provided they have not been alerted for combat duty or are not serving in a hazardous area. The Military Personnel Financial Services Disclosure must be submitted with the application. Required Forms: 200-673 (UHL); 18-673 (UFFL).

Stranger-Owned Life Insurance (STOLI)

Applications will not be accepted if any agreement or understanding exists that provides for any party to obtain an interest in any policy issued on the Proposed Insured who does not have an insurable interest in the life of the Proposed Insured.
